CalSCHLS Surveys

Mountain View fifth and seventh grade students will be participating in the California Healthy Kids Survey. This is a comprehensive youth health risk and resilience data tool for schools. The survey is developed by WestEd, a non-profit educational research agency, under contract with the California Department of Education. You will receive a letter containing more detailed information during Parent/Teacher conferences, September 22-26, 2025. The survey is administered online and is anonymous. No names will be attached to any surveys and the information is used for analysis only under strict confidentiality controls.
 
Students will be taking the survey at a date and time determined by their teacher during the survey window of February 3-5, 2026.

Parents may access the California School Parent Survey by clicking on the links below.  Please take the survey under the school of your oldest child in Mountain View School District. You do not need to repeat the survey for each school that your children attend, but you are welcome to do so if you choose.
 
This is an anonymous survey. It is designed so that no one can be identified from the data. You do not have to respond to any questions you feel could be used to identify you personally. Simply skip any such questions.

CHKS SCHOOL STAFF SURVEY
 
Mountain View School District staff members may access the California School Staff Survey by clicking the link below that correlates with the school site at which you work. The purpose of this survey is to obtain staff perceptions on a variety of areas as they relate to student well-being and learning.

Restraint and Seclusion Data

Data Collection

The CDE collected restraint and seclusion data for the first time from local educational agencies (LEAs) as part of the 2019–20 CALPADS End-of-Year (EOY) 3 annual data submission. In preparation for this data collection, the CDE directed LEAs to start collecting these data locally beginning in fall 2019 for submission to CALPADS from May to September 2020. To accommodate the collection of these data, the CALPADS student discipline data files were modified to include the collection of the following behavioral restraint and seclusion data, which are defined in EC section 49005.1:

  • Mechanical Restraint: Use of a device or equipment to restrict a pupil's freedom of movement (with exceptions).
  • Physical Restraint: A personal restriction that immobilizes or reduces the ability of a pupil to move his or her torso, arms, legs, or head freely (with exceptions).
  • Seclusion: The involuntary confinement of a pupil alone in a room or area from which the pupil is physically prevented from leaving.
