Keeping Your Child’s Medical Information Updated
To ensure we can care for your child safely and efficiently, families are asked to:
- Update medical information every school year
- Notify the health office immediately when health changes occur
- Provide completed medical forms for medications, chronic conditions, or special accommodations
- Submit medication with the required forms directly to the health office
- Ensure the school has current emergency contact information
Accurate information helps us prepare for emergencies and support your child’s health needs.
We’re Here for You
