Health and Immunization Resources

Medications

If your child needs to take any medication at school—prescription or over-the-counter—please follow the steps below:
  1. Medication Authorization Form:
Have your child’s healthcare provider complete the Medication Authorization Form (linked below). This form provides the necessary instructions for safe administration at school. All sections must be fully completed, including the PRN (as needed) symptoms—for example: cough, wheezing, hives, fever of 100.0°F, pain, headaches, or any other symptoms that indicate when the medication should be given.
  2. Parent/Guardian Signature:
Parents/guardians must also sign the form. Your signature gives consent for school staff to administer the medication as directed.
  3. Submit Medication and Form to the Health Office on the first day of school or after:
Bring the medication in its original container along with the completed Medication Authorization Form to the health office. Both the form and the medication must be submitted together for the medication to be accepted. If the original container is unavailable, please visit your pharmacy and ask for a new patient label to be provided.
Important Notes:
  • A new Medication Authorization Form is required each school year.
  • Accurate and up-to-date information helps us care for your child safely.
  • Medications cannot be administered at school until all required documentation is submitted.

  • Medical Procedures: If your student is in need of specialized physical health care services (i.e. catheterization, nebulizers, tracheostomy, etc.), contact the district nurse at (909) 947-2205.

Restraint and Seclusion Data

California Department of Education

Data Collection

The CDE collected restraint and seclusion data for the first time from local educational agencies (LEAs) as part of the 2019–20 CALPADS End-of-Year (EOY) 3 annual data submission. In preparation for this data collection, the CDE directed LEAs to start collecting these data locally beginning in fall 2019 for submission to CALPADS from May to September 2020. To accommodate the collection of these data, the CALPADS student discipline data files were modified to include the collection of the following behavioral restraint and seclusion data, which are defined in EC section 49005.1:

  • Mechanical Restraint: Use of a device or equipment to restrict a pupil's freedom of movement (with exceptions).
  • Physical Restraint: A personal restriction that immobilizes or reduces the ability of a pupil to move his or her torso, arms, legs, or head freely (with exceptions).
  • Seclusion: The involuntary confinement of a pupil alone in a room or area from which the pupil is physically prevented from leaving.

The chart below reflects data for the 2024-2025 school year.
